PREPARING FOR YOUR TOUCH-LESS VISIT
IN 3 STEPS

Below are the steps to ensure an efficient, touch-less visit. Let's get started.
1. MAKE YOUR APPOINTMENT
Save time by completing a quick appointment request (Blue button on bottom right) or call the office for an appointment. A list of participating insurances can be found here.
After filling out the appointment request form, You will received two emails: one confirming the request and another email confirming the appointment.
2. USING THE PATIENT PORTAL
Access your healthcare portal once your appointment is confirmed to verify your personal medical information, pick a preferred pharmacy and sign mandatory consents.
New patients- you will receive an email from email@example.com with easy instructions on how to set up your portal access.

Establish patients- sign into the portal using your chosen username and password.
Never used the portal? Search for your welcome email from firstname.lastname@example.org.
Can't find the email? Email or call our office to trigger a new unique email link to your portal.
Forgot your username? Use this email link Lincoln Road Dermatology to retrieve your previously chosen username.
Pick your preferred pharmacy located in the head (profile) icon.
DON'T FORGET TO SIGN YOUR CONSENTS....SOME ARE MANDATORY TO ESTABLISH CARE
Unfortunately consents can only be signed from a computer or iPad. They are located under the Appointment tab. Please sign consents only in your preferred language.
New consent forms may populate due to change in office and statewide policies.

3. INTAKE & TOUCH-LESS PAYMENT
Ideally, 24-48 hours before your anticipated visit, we will confirm that your insurance is active. You should fill out your intake information through Sadio, our secure, patient healthcare portal so that your visit can occur promptly at your scheduled time. If you cannot do this, please arrive 10-15 minutes before your scheduled time.
Opting for credit care on file via Square enables touch-less payment of any balances or extra costs incurred. To learn more about touch-less pay, click below.
Finishing steps 1-3 before your arrival will facilitate a quick transition from the front door to your exam room.
